Publication of Scottish Executive (2004) Investigations into Scottish Borders Council and NHS Borders Services for people with learning disabilities: Joint Statement from the Mental Welfare Commission and the Social Work Services Inspectorate. This followed on from inquiries by the Social Work Services Inspectorate and the Mental Welfare Commission into the case of a 30-year-old woman, referred to as 'Miss X' for legal reasons.

The case emerged in March 2002 after Miss X was admitted to hospital suffering multiple injuries from physical and sexual abuse. Police investigation revealed she had been subject to a series of abuse and assaults over the previous weeks and possibly much longer. In August 2002 three men, including a carer, were convicted on assault charges, and were sentenced at the High Court in Edinburgh to between 7 and 10 years' imprisonment.
